The state of Texas requires that all students with Individual Education Plans (IEPs) are provided an ongoing, custom designed plan of action from high school through post-secondary education, job placement, and/or living arrangements according to their needs and interests from ages 14 - 24. This plan of action, which changes as the student’s (or adult's) needs, capabilities, and interests change, is known as a Transition Plan, providing the basic structure for preparing individuals to live, work, and play in their community as fully and independently as possible. (PACER. 2001).
We have provided this website as a resource tool for parents and students to find information regarding contact information, services offered, and publications from public and private agencies that may offer the assistance needed for various daily living, career and/or educational needs. This resource is not intended to be all-inclusive nor provide legal, financial, or counseling advice. However, we do hope it will be a useful, one-stop location for searching needed local, regional, and national information pertaining to transition services available to Texas residents.
